Infertile to I'm Fertile Enrollment Agreement:

This Enrollment Agreement (“Agreement”) is entered into between Health Made In America, LLC d/b/a A Teaspoon Of Wisdom ("A Teaspoon Of Wisdom") and the individuals (“Client” or “Participating Partner”) enrolling in the Infertile To I’m Fertile Program ("Program")

Program Description:

Our fertility Program “Infertile To I’m Fertile” is a highly personalized fertility education and wellness consulting Program provided by A Teaspoon Of Wisdom. It is designed to optimize fertility for both men and women.

The Program is educational and intended to support Clients and Participating Partners in making informed health and wellness decisions. The Program does not provide medical diagnosis or treatments.

The Program includes:

Comprehensive intake review
Personalized fertility optimization plan
Nutrition and lifestyle recommendations
Scheduled consultations
Follow-up support
Education in fertility optimization
Supplemental education
Support for both partners, male and female

Support will last 12 months or until conception, whichever comes first. Options for post-conception education and support will be given during final call after conceiving.

The Program will include 7 total calls. The initial call after the comprehensive intake forms are completed by both partners will be 90 minutes, and is intended for both, Client and Participating Partner, to be on the call. The personalized fertility optimization plan will be delivered approximately 7 days after the initial call. Two weeks after the personalized fertility optimization plans are delivered, there will be a follow-up call, approximately 30 minutes, to ensure a complete understanding and implementation of the plan. Follow-up support calls (15 to 30 minutes) will then occur at 2 months, 3 months, 5 months, 8 months, and 11 months from the start of the Program (completion of comprehensive intake forms). There will be one additional call after conception or 2 weeks after the Program has ended for additional support.

Access to support in-between calls will primarily be done through text message or email, which will be provided after the start of the Program. Additional calls can be requested if the nature of the call cannot be properly done by text or email. Access is generally unlimited within reason, determined by A Teaspoon Of Wisdom.

Payment Terms:

The Client agrees to pay the fees selected at enrollment. The fees include a comprehensive intake review, personalized fertility optimization plan, and continued support for both, Client and Participating Partner. An initial payment is required prior to Client receiving the comprehensive intake form and review.

All payments are due according to the selected payment schedule.

If a scheduled payment fails and becomes greater than 30 days overdue, services will be suspended and the money-back Guarantee is permanently voided, as outlined in Pregnancy Guarantee & Refund Policy. Services will resume after payment is received, but money-back guarantee remains voided.

The Client is responsible for maintaining valid payment information throughout the Program.

Payment Responsibility: The individual designated as the Client is responsible for all payment obligations associated with enrollment in the Program. Both participating individuals acknowledge and agree to the terms of this Agreement.

All prices, fees, and transactions stated on this website or processed through our payment gateway are in U.S. Dollars.

Pregnancy Guarantee & Refund Policy:

Pregnancy Guarantee:

A Teaspoon Of Wisdom offers a money-back pregnancy guarantee (“Guarantee”) with our Program “Infertile To I’m Fertile” which is subject to the conditions outlined below.

To remain eligible for the Guarantee, the Client must participate for 12 months and meet the following requirements:

Complete all required intake forms.
Both, Client and Participating Partner, need to attend the initial 90-minute call.
Each partner (Client and Participating Partner) must be present on at least 50% of the follow-up calls.
Must remain actively enrolled, engaged, and an active participant for the full Program duration of 12 months.
Notify A Teaspoon Of Wisdom of health changes or significant fertility-related developments.
Payments for the Program fee cannot exceed greater than 30 days overdue.
If, after 12 months, there is no conception, the Client is eligible for their full money back that the Client paid for the Program fee.

Participation Requirements:

The Guarantee is intended for Client and Participating Partner who actively participate in the Program. Eligibility for the Guarantee may be affected by repeated missed appointments, no-call/no-shows, excessive cancellations or rescheduling, failure to complete requested check-ins, extended periods of non-communication, failure to provide requested information, or other significant failures to participate in the Program.

Eligibility will be evaluated based on the Client’s and Participating Partner’s overall level of participation and substantial compliance with Program requirements. A Teaspoon Of Wisdom may review relevant Program participation records when evaluating eligibility under the Guarantee.

Requests to receive money back under the Guarantee must be submitted via email to james@ateaspoonofwisdom.com within thirty (30) days following completion of the Program.

Guarantee Exclusions:

Exclusions from the Guarantee will be determined by A Teaspoon Of Wisdom and will always be communicated to the Client. These exclusions may include, but are not limited to the following:

Surgery on reproductive organs.
Surgical removal of organs/tissues that result in the alteration of hormones.
Known tubal obstructions
Continued use of hormone altering medications

If any exclusion arises between the initial 90 minute call and delivery of the personalized fertility optimization plan, the Client will be notified and have 2 options:

Continue the Program without the Guarantee.
Receive a full refund of deposit or payment made and end the participation in the Program.

For purposes of this Guarantee, pregnancy is defined as a confirmed positive pregnancy test during the Program or within twenty (20) days of the completion of the Program.

The Guarantee is subject to all terms contained within this Agreement.

Refund Policy:

We encourage prospective Clients to carefully consider their decision before enrolling. Due to the personalized nature of the Program and the resources dedicated to each Client upon enrollment, Program fees are generally non-refundable. Enrollment secures your place in the Program and allows us to begin preparing individualized support. Except as provided under the Pregnancy Guarantee, Program fees will not be refunded for changes in circumstances, scheduling challenges, or discontinuation of participation.

Any refund approved under the Guarantee will be issued within 30 days of approval. Refunds may be issued by company check, ACH transfer, or another payment method selected by A Teaspoon Of Wisdom. Refund is for full Program fee amount paid by the Client.

Educational Disclaimer:

A Teaspoon Of Wisdom provides educational fertility consulting designed to help Client and Participating Partner better understand factors that may influence reproductive health, fertility, nutrition, lifestyle, and overall wellness. The Program is intended to support informed decision-making and personal responsibility and should not be interpreted as medical advice, diagnosis, treatment, or healthcare services.

Although the Program recommendations may include discussion of nutrition, lifestyle practices, supplements, laboratory testing, medications, and fertility-related topics, all healthcare decisions remain solely between both, Client and Participating Partner, and their qualified healthcare providers.

Participation in the Program does not establish a physician-patient, pharmacist-patient, practitioner-patient, or other healthcare provider relationship. A Teaspoon Of Wisdom does not diagnose, treat, prescribe for, or manage medical conditions through the Program.

Client and Participating Partner acknowledge that they remain responsible for their own healthcare decisions and agree to consult their physicians or other qualified healthcare professionals regarding any medical concerns, symptoms, diagnoses, medications, treatments, fertility interventions, or changes to their healthcare plan.

Assumption Of Responsibility:

The Client and Participating Partner acknowledge that meaningful improvements in health and fertility often require personal commitment, consistency, and active participation. A Teaspoon Of Wisdom provides education, guidance, resources, and recommendations. The implementation of those recommendations, however, remains solely the responsibility of the Client and Participating Partner.

The Client and Participating Partner understand that outcomes may be influenced by numerous factors beyond A Teaspoon Of Wisdom’s control, including medical history, lifestyle, environmental factors, partner-related factors, and adherence to the Program recommendations.

By participating in the Program, the Client and Participating Partner accept responsibility for their own decisions, actions, and level of participation and acknowledges that A Teaspoon Of Wisdom cannot make decisions on the Client's and Participating Partner’s behalf.

Limitation of Liability:

The Client and Participating Partner acknowledge that fertility, reproductive health, pregnancy, and related outcomes are influenced by numerous biological, medical, environmental, genetic, and lifestyle factors beyond A Teaspoon Of Wisdom’s knowledge or control. Accordingly, A Teaspoon Of Wisdom makes no representations or warranties regarding any specific health, fertility, pregnancy, or reproductive outcome.

To the fullest extent permitted by law, A Teaspoon Of Wisdom shall not be liable for any loss, injury, claim, expense, damage, or adverse outcome arising from participation in the Program, reliance upon Program materials, implementation of recommendations, decisions made by healthcare providers, laboratory findings, pregnancy outcomes, miscarriage, or any related circumstance.

In no event shall A Teaspoon Of Wisdom’s total liability exceed the Program’s fee paid by the client for participation in the Program.

Cancellation and Rescheduling Policy:

A Teaspoon Of Wisdom values the time and commitment of both, Client and Participating Partner, and practitioners. Because Program sessions are reserved specifically for each Client and Participating Partner, we ask that Client or Participating Partner provide as much advance notice as possible when a scheduling change is needed.

Clients and Participating Partners may reschedule or cancel scheduled appointments by providing at least 24 hours' notice. A reasonable effort will be made to accommodate rescheduling requests based on availability.

A pattern of missed appointments, no-call/no-shows, or repeated late cancellations may impact Program participation and may be considered when evaluating eligibility under the Program Guarantee.

A Teaspoon Of Wisdom understands that unexpected circumstances may occur. Requests for exceptions due to emergencies or unavoidable situations will be reviewed on a case-by-case basis.

A Teaspoon Of Wisdom reserves the right to reschedule appointments when necessary and will provide reasonable notice whenever possible.

Communication Policy:

A Teaspoon Of Wisdom is committed to providing thoughtful support and timely communication throughout the Program. Primary communication between scheduled Program calls will occur through email and text messaging.

A Teaspoon Of Wisdom will make reasonable efforts to respond to Client or Participating Partner communications within 48 business hours during regular office hours, Monday through Friday, 9:00 AM to 5:00 PM. Communications received outside of office hours, on weekends, or during holidays will be addressed during the next available business hours.

Scheduled Program calls will be arranged based on mutual availability and may occur outside of regular office hours when appropriate.

Clients and Participating Partners are encouraged to use scheduled calls for in-depth discussions, complex questions, and topics requiring additional guidance. Text and email communication are intended for follow-up questions, clarification, updates, and ongoing support between scheduled sessions.

A Teaspoon Of Wisdom does not provide emergency medical support or urgent medical advice through text or email. Clients and Participating Partners should contact their healthcare provider or emergency services for urgent medical concerns.

Confidentiality:

A Teaspoon Of Wisdom is committed to protecting the privacy and confidentiality of information shared by Client and Participating Partner during the Program. Client and Participating Partner information may include, but is not limited to, fertility history, reproductive goals, lifestyle information, nutrition habits, wellness information, personal circumstances, and other information voluntarily provided by Client or Participating Partner.

A Teaspoon Of Wisdom will use Client or Participating Partner information only for the purpose of providing Program education, guidance, communication, and support. Client or Participating Partner information will not be disclosed to third parties without Client or Participating Partner consent, except where disclosure is required by law or reasonably necessary for business operations, payment processing, legal compliance, or protection of rights and safety.

Both, Client and Participating Partner, acknowledge that information shared during joint sessions may be accessible to both participants. If either partner wishes to discuss information privately, they may request an individual conversation when appropriate.

Clients and Participating Partners understand that communication through email, text messaging, video platforms, and other electronic methods may involve privacy risks. By participating in the Program, Client and Participating Partner consent to communication through these methods while acknowledging that no electronic communication system can be guaranteed to be completely secure.

Dispute Resolution:

A Teaspoon Of Wisdom values open communication and encourages Clients or Participating Partners to address any questions, concerns, or disagreements directly with A Teaspoon Of Wisdom in an effort to reach a reasonable resolution.

Before initiating formal legal action, Client, Participating Partner, and A Teaspoon Of Wisdom agree to make a good-faith effort to resolve any dispute, claim, or disagreement related to the Program, this Agreement, or the services provided.

If the matter cannot be resolved through good-faith communication, either party may pursue any remedies available under applicable law.

This Agreement shall be governed by and interpreted according to the laws of the State of Illinois.

Unforeseen Circumstances:

A Teaspoon Of Wisdom understands that unexpected circumstances may occasionally affect the delivery of Program services. If circumstances outside of A Teaspoon Of Wisdom's reasonable control occur, including illness, emergencies, technology disruptions, or other unforeseen events, A Teaspoon Of Wisdom will communicate with Client or Participating Partner and make reasonable efforts to reschedule sessions, adjust Program timelines, or make other appropriate adjustments.

Such circumstances will not be considered a failure by A Teaspoon Of Wisdom to provide Program services, provided that A Teaspoon Of Wisdom makes reasonable efforts to fulfill its obligations under this Agreement.

Any impact on Program eligibility, participation requirements, or the Guarantee will be evaluated according to the applicable terms outlined in this Agreement.
